7/26/18 Oncology Visit

Vitals

  • Weight: 168.4, last time was 181lbs
  • Blood pressure: 131/70
  • PSA level: 486, last time 407.8
  • LDH: 318 (LDH level indicates tumor activity, should be 121-220)

Reviewed / Discussed Chemotherapy

  • Will only be 1/3 of the usual dose due to age
  • Should not affect physical abilities
  • Will loose hair
  • Some fatigue, possible nausea
  • May cause dry eyes
  • Will be every 3 weeks as tolerated
  • Goal is to help prevent shortness of breath and any pain, not curable
  • Options at this point are chemo or hospice
  • After 4-6 months of chemo, may have some tingling in fingers
  • Within first 3 days of treatment may feel nausea, after 1.5 weeks may feel fatigue indicating white blood cell count is low
  • Will do regular blood work and urinalysis to check for infections and white blood counts
  • Will recommend/order labs to be done at home
  • Prior to starting chemo, will need a urinalysis to check for any UTI, will treat to resolve prior to starting chemo
  • From today it will be at least 2 weeks for approval before can start chemo
  • Grandpa wants to try chemo at this time
  • Needs bone transfusion prior to chemo, cant do during chemo

Face Rash concerns

  • Face rash – possible acne infection, cream doesn’t seem to be working. Doctor recommends to discontinue.
  • Took pictures of rash, will send to dermatologist friend to see if consult needed or recommended treatment
  • She will order consult if necessary
  • Anita will call doctor office with name of cream (Flurorouracil)
  • Do not use until further notice

PLAN

Doctor will order:

  • Urinalysis and urine cultures ASAP
  • Usual PSA/LDH levels for prior to last visit
  • Bone infusion ASAP
  • Chemotherapy (may take 2 weeks for approval, if copay issues they will help find foundations for payment)
  • Routine WBC and UA lab work to be done at home
